- QUESTION: Getting a close look at the skin on your teen age son is pretty
- difficult for a doting mother, but when he ran around the house without shoes
- and socks on I noticed some peculiar marks on his heels. They looked like
- dark spots, and I was afraid they were a kind of cancer, like the kind you can
- get from the sun. It took a lot of doing to get my son to the doctor, but
- then the only report I got was that there was nothing to worry about, it was a
- "black heel". Please tell me what this means.

- ANSWER: The dark spots that can be seen on the heel of young athletes
- frequently give a moment of anxiety when they are mistaken for a melanoma.
- Melanomas are dark, pigmented skin lesions, most often found on parts of the
- body exposed to intense or prolonged sunlight, that are cancerous, but a
- "black heel" (or "talon noir" from the French) is fortunately a very benign
- finding. There are no symptoms from a black heel, which can be seen as small,
- dark, dot-like spots forming an irregular pattern on the heel. In medical
- terms they are small "petechiae", or tiny blood clots under the skin. They
- are caused by the friction of athletic shoes against the skin of the heel
- during abrupt stops that occur during athletic competition, particularly in
- basketball. There is no treatment needed and adding additional padding to the
- heel of the shoe, to hold the heel more firmly in place, can prevent these
- innocent spots from occurring.
